Ok, there are three parts of a potion: the water-filled vial, the herb, and the secondary Ingredient. To make a potion, use the herb on the water filled vial, then use the secondary ingredient on the Unfinished Potion you will get. You will then get your potion.
To start making potions, you have to do the quest called Druidic Ritual. You can start it by going to tavlerey and speaking to Kaqemex who is just north of the town at an altar where there are some druids and a NPC. For more help go read the Druidic Ritual guide.
Here is a list over potions and herbs which you need the following lvls to make.

Places
Here are the best places and monsters to kill to get herbs. They are generally Choas Druids, though I suggest the Dungeon North of yannile if you have an OK Combat lvl.

This is all that you need to make a potion. In the future, solinent will add a new section, called herbs. This section will have the herb experience you get, and have all the herbs and lvls to ID
